A lack of local anaesthetic was a problem until cocaine and its derivatives showed up, but nitrous oxide (laughing gas) had been discovered in the 1840s, though from the information here it looks like it wasn't in general use until the Civil War era. There were also ether and chloroform, both widely available among military doctors during the war. So one wouldn't need to be conscious during the extraction, though I have a feeling that for an ordinary one, a dentist might try to do it so fast the procedure would be over before the patient could hardly react. Then more general anodynes like laudanum would do some good.
That bendy one at the end for molars looks like you could get a good grip for a quick pull. I am sure they did not treat for an abscess. The answer was to pull quickly.
