I found the following blurb in the Cincinnati Daily Gazette, April 8, 1881:

"In two places in the columns of the Gazette of yesterday it was stated that Gen. McPherson was killed at Shiloh. As a matter of fact, the lamented soldier was killed two years later between Resaca and Atlanta. The unveiling of a statue of the General on the anniversary of the battle of Shiloh was the cause of confusion in the reportorial mind."
 
One of the most famous goofs.

"Confusion in the reportorial mind".... I love that. Among the various news outlets available to me, I could make that statement nearly every day. ...and I probably will.
