Missing Confederate Officer Rank

In the lists of Confederate Officer insignia, they invariably leave out the lowest rank: 3rd Lieutenant. I believe it was according to Hardee's Manual the 3rd Lt was in charge of the reserves. I have not been able to verify but have some clues that the rank insignia for 3rd Lt was a blank collar on an officer's jacket. Anybody able to help with this?
 
In the enlisted ranks there were sometimes men designated as Second- and Third Sergeants, or Second- and Third Corporals. I believe this was done to keep the seniority straight, but had no particular insignia.
 
I believe it's the same with a "High Private". There isn't any increase in pay grade, it just designates someone who has more experience and responsibility.
