Looking for a little help.

Hello all,
I recently was given a pdf copy of my Great Grandfathers records from the National Archives.
One of the records titled "Memorandum From Prisoner Of War Records" has bleed-through from the other side of the original form, and also has writing crossed out and changes written in. Part of the writing is easily read but other parts are pretty confusing.
I am hoping that some of you who have experience reading the hand writing on old documents might help me to make sense of this document.

I'll give you what I can figure out top to bottom starting at Captured at:

Guntown Mississippi June 12th, 1864

Confined at Andersonville ? and ? to Colonel O'Neil a rebel recruit...

Escaped from (Savannah Ga April-scratched out) and in union lines Savannah

Transferred from Annapolis, Md to Camp Chase, April 6, 1865 where... ? May 1, 1865

To Pen (Pension ?) office March 7, 1887

That's all I could decipher, good luck.
