Help with Civl War document

I need help with the attached Civil War document. I can't make out all the words. Any help will be greatly appreciated! I think it says:

This Officer has been ___ by Colonel Hays for a flagrant breach of Military Discipline while the Regiment was on Detached Service at "______" _____ _____.

"This Officer has been [...Indicted?...] by Col. Hays for a flagrant Breach of Military Discipline while the Regt. was on Detached Service at "Richards" ford last week." He possibly didn't know the ford's real name, thus the 'Richards' in quotes.

What's the officer's name and unit? You might be surprised at what some people here can dig up.

What would a soldier have to have done to be sited for a "flagrant breach of military discipline'?
 
The regiment was the 8th Pennsylvania Reserves, Colonel George S. Hays commanding. The note was written by then BG John Fulton Reynolds (later commander of the 1st Corps; killed at Gettysburg on 1 July 1863), commander 1st Brigade Pennsylvania Reserves. The other name of note is BG George A. McCall, commander of the Pennsylvania Reserve Corps (actually a division of 13 regiments in three brigades). The date of May 17, 1862, corresponds to the time when Reynolds' Brigade was advancing from Washington to Fredericksburg VA prior to the Seven Days Campaign.
