Help deciphering

Researching one of my relatives who fought with the 50th Virginia Infantry Regiment (Anderson Stapleton). My understanding is that he was wounded at Chancellorsville.

I found this hospital register from Howard's Grove in Richmond. I see where is states "left shoulder" but I cannot make out the words preceding it. Two words (abbreviations?) with colons.

Can anyone help? Thank you

Its the medical term for gunshot wound.
