I am looking for the information that was filled out for enrollment into the Confederate army. Of course, name, age, birthdate? birth location? next of kin? marital status, wife? address? etc.
Does anyone have this information in general?

I am looking for this information for GGG grandfather B. P. Bickham, he was killed on Nov 4, 1862 at Vicksburg. He was a member of Co. F, LA 17th Infantry. He was from the Shreveport, LA area. In 1860 LA Census. I know he enlisted on Oct. 16, 1862. So he only made 20 days per card 78289883 from www.fold3.com
Thanks in advance

B P. Bickham : Enlisted and mustered on 10/1/1861 as a Private into "F" Co. LA 17th Infantry. He died of disease on 11/4/1862 at Vicksburg, MS. Buried: Cedar Hill Cemetery, Vicksburg, MS.

If you have access to his record at fold 3, it will have all the info that's available. Confederate service records, at times, recorded a physical description of the soldier (age, complexion, height, and color of eyes and hair. Little else) Sometimes the distance in miles from their home to the place of enlistment. Different States usually recorded different info. If B.P. was married, perhaps his widow later applied for a pension. Louisiana Confederate Pension applications can be accessed here : https://www.familysearch.org/ all that's needed is you creating a free account.

Thanks for the reply.
Seems to be confusion on his enrollment date. May have to look at originals. She did not file for a pension. Possibly may have been dead by 1898. I appreciate your response.
