Assistance Transcribing Pension Record

This is the pension record of Pvt. James H. White, Company "D" 26th Virginia Infantry Regiment. I'm having trouble reading his incredibly small handwriting.

I'm specifically interested in the nature of his wound that he received at Bermuda Hundred on June 9th, 1864.

Any help?

I'm not very good at transcriptions.
 
It appears James was severely wounded and was affected for the rest of his life. It is tough to decipher, appears to be the handwriting of the person certifying the statement. I tried my best, hope it helps. He was wounded in a skirmish 9 Jun 1864.
- Wound under the skin / chin and in the neck, affecting?
- I have a constant soreness in my head, can eat nothing but soft food, and am affected with dizziness
- 3rd - The bullet entered in the skin / chin, fractured? the lower right jaw bone and passed out on the right side of the neck, near the back of the neck, injuring the throat
- 5th - Soreness in the throat, cannot swallow without pain, cough with pain pain in jaw and throat, dizziness, pain in right shoulder

James was in Wise's Brigade, as were many of my relatives who were in the 34th Va. Infantry. Those old vets were tough.

I was wounded in a skirmish at Bermuda Hundred, near Petersburg, on the south side of James River, by a bullet under the [skin? - probably chin] and in the neck, affecting

[He gives the date of his wounding as June 9th, 1864]

The bullet entered in the -----, fractured the lower right jaw [and] bone and passed out on the right side of the neck [over? or near?] the back of the neck, injuring the [throat?]

Soreness in the throat, cannot swallow without pain, cough [under?] pain, pain in jaw and throat, dizziness, pain in right shoulder

Cannot stand exposure to the -----, pain and stiffness in neck and right shoulder, rendering manual [labor?] totally ------

My people were with the 26th. Although James White is not a relative.
