South Mountain Sept. 1862. What were the three gaps the AOP attacked?

Bonus: I won a Bronze Medal at the 1912 Olympic Games for my horsemanship. A decorated military officer, I was Commandant of Cadets at West Point and served as Chief of Cavalry. Who am I, and what was my connection to the Civil War?

Birthday Bonus:
I was born in Washington county Pennsylvania in 1821. I was a private in Company B, 73rd Ohio Volunteer Infantry. On July 2, 1863 I was mortally wounded during the battle of Gettysburg. That night my cries of pain were such that it inspired a Musician to crawl out and drag me to safer ground earning him a Medal of Honor. I died on July 10, 1863 and I am now buried in the 4th row, 4th from the right grave in the Ohio section of the Gettysburg's Soldiers' National Cemetery.

In July 1953, I was honored by having my famous Great Grandson visit my gravesite. I invite you to visit my grave on your next Gettysburg visit.

Who am I and who is my famous Great Grandson?

John Carter Montgomery (1881-1948) won Bronze in the team equestrian event. Montgomery was a 1903 West Point graduate and an officer in the 7th Cavalry. His wife was Virginia Lee, daughter of rebel Major General Fitzhugh Lee.

George Nixon, Private, 73rd Ohio Volunteer Infantry, grandfather of President Richard Nixon. The musician who rescued him was Richard Enderlin (1843-1930)

Bonus: General George S. Patton, and his grandfather was a colonel for the CSA (Colonel George S. Patton Sr.)
Birthday Bonus: Private George Nixon, his Great Grandson is the 37th President of the USA, Richard M. Nixon

Edit - Patton participated in the 1912 Olympics, but did not win a medal.
