Doesn't look like there is any catastrophic wound to the patient's arm. Let's hope the surgeons aren't going to amputate just for practice. Looks like the patient may be still awake?

The surgeon standing left holds the saw at the ready. The surgeon in the middle seems to be about to administer the chloroform or ether via the device held up near the patient's face. Here's an enlargement of that part of the image.
