Reading Cursive Help

Starting to work on a bit of a research paper I might publish later on. My old USCT project, as some may remember -- I might start posting screenshots of illegible text here, in case anyone can help:

First off, this for Maj. Zuinglius McKay, 86th USCT:

What I can read:

Requirements of law not fulfilled [I believe this refers to McKay possibly not yet being versed in military affairs, being what appears to be a civilian appointee.]

_____ _____ except _____ cancelled by ___, one roll. [Based on the context, I assume this has something to do with the muster roll itself, but have included it here in case it is important.]

Thoughts?

I read

"Requirements of law not fulfilled"

"Entire entry except remarks cancelled by line, on roll"

But I am often wrong and do not know if this makes sense or what it could possibly mean when you do figure it out please let us know!
