Killed or Captured?

In an effort to locate a grave, I obtained copies of military and pension records for my 2g-grandfather. I believed he was killed at Fisher's Hill. The records seem to be contradictory.

First a casualty sheet. Next is the POW memorandum (additional information about this form would be appreciated). Then a letter from the AG seeming to refute a pension, followed shortly thereafter by one from the SG confirming KIA. I'll be grateful for any guidance provided.

I apologize for being so clumsy at posting. Hopefully I will improve soon and will rely on your patience and kindness in the interim.

I have looked at some records were the soldier was listed as MIA and they filled out a Casualty Sheet that listed him "Assumed Dead". Then later, the soldier either returned to his unit or was proven to be living when he applied for a Pension.
 
The document AG4-19-1865 states he was dropped from Roll but there was No Evidence of Death.
Then the SG5-12-1865, which was dated a few weeks later, seems to be confirming that he died in battle.

And one question is how did they know that. I don't know how POWs were reported. Is that what the POW memorandum tells me and what is the source? Does that document that he died while a prisoner? I wish I could decipher the notations on that document better - my guess is that one entry is "died" and some refers to a "volume" in a source document. The long notation at the bottom is a mystery to me.
