From the apparent black facing on his frock coat he may well have been a Confederate surgeon, which position carried the rank of major; if so, @lelliott19 is the person here most likely to be able to possibly recognize him.

That fits with James N. thinking he was a CSA surgeon. Are there any markings on the frame or back?

I put the painting through civil war photo sleuth and this is one of the result. The result was Ambrose Hill, he fits the rank description, hope it helps.

Sorry - it's NOT A. P. Hill, who was a colonel only a very short time and never in the CSA Medical Department.
