The National Union Party, the Democratic Party the main two there was a third the Radical Democratic Party.

Edit - The question asked for two parties and it is clear that your answer identifies the National Union and the Democratic as the two main parties on the ballot, so you will get credit for a correct answer.

You are correct that a third party, the Radical Democracy Party, nominated a Presidential candidate in 1864, although that candidate (Fremont) withdrew from the race in September. I am unable to confirm whether the party's name actually appeared on the Presidential ballot in November.
