Ride, ride, ride
In the Summer of "63 a diversionary plan was being hatched. Orders were cut to stay within the next state only. Myself and my men mounted up to do what we must, and rode off in a cloud of dust.
Ride, ride, ride.
We crossed the state line and havoc we created. We hit stores and supply depots, Communications and track were at our mercy. The men in Blue were not pleased with our acting. They chased and skirmished whenever they could in hopes to send us packing.

Another state line lay just yonder ahead. Orders were for thee, not for me. Onward they went. 2500 horses were taken, 6000 prisoners had been captured and paroled, they never suspected we would be this bold,

Another border we crossed. Skirmish and skedaddle. A total of 34 bridges we left in disarray. More men in Blue joined the pursue. We did what damage we could and rode hard for the state line. The river fords were flooded so we helped ourselves to two steamboats to get across. One we torched and the others sent downriver with orders not to look back. Our actions had left local economies in disaster. We had pushed our luck beyond what was prudent. A battle ensued, outnumbered we were as the Blue Army caught up, so surrender we must. Some got away, but for me it was off to the State Pen. Escape was foremost in our minds. When the opportunity arose we wasted no time and skedaddled for home.

1. John Hunt Morgan(1825-1864);
2. Tennessee, Kentucky, Indiana, and Ohio;
3. Morgan was captured by local militia commanded by Captain James Burbick at a site 0.4 mile between West Point and Gavers, Columbiana County, Ohio, July 26, 1863.
Morgan later 'officially' surrendered his command to Maj. George W. Rue at a site closer to West Point, marked by a monument and cited as the "farthest point north ever reached by any body of Confederate troops".
Both sites are near New Lisbon, Columbiana County, now called Lisbon, which is sometimes cited as the site of both Morgan's capture and surrender.

Edit - The second part of the question asked for four states. The ride started in Tennessee (though they didn't do any raiding there) and then went through Kentucky, Indiana, and Ohio, with some, though not all, of Morgan's men making it into West Virginia, so a case can be made that they actually rode through five states. I'm accepting answers that name any four of the five.

Some sources say the capture took place in Salineville, some say West Point. If I do a search for West Point, Ohio, I am directed to New Lisbon. So I'm accepting any of those three answers to the third part of the question.
