The twisted piece is a hook that was attached to a sword belt ring - swords were most commonly suspended from a waist belt by two long leather straps, one hanging from the wearer's left side; the other hanging from the middle of the back. If left that way, the tip of the scabbard would likely DRAG on the ground and get in the wearer's way, possibly even tripping him. The loop on the hook held it to the same left-side ring on the belt that the strap was suspended from; the wearer could lift his sword and slide the top scabbard ring onto this hook, thereby suspending the sword off the ground. @JOHN42768 is correct as to the finial, making it likely that BOTH date from the war.
 
It is indeed a sword hanger, the finial being brass is more than likely yankee
