1. What is this brigade with it's state name ? The First Vermont Brigade ("Old Brigade"). Organized in October 1861; comprised of the 2nd, 3rd, 4th, 5th and 6th Vermont Infantry Regiments. After the Battle of The Wilderness (May 5-7, 1864), the 1st Vermont Heavy Artillery Regiment (formerly the 11th Vermont Infantry) was added to the brigade.

The Brigade suffered the highest combat fatalities of any brigade in the history of the United States Army with some 1,172 killed in action.
At the Battle of The Wilderness, losses by the brigade totaled 1,269 killed, wounded, and missing in less than 12 hours of fighting. After Cold Harbor (June 1, 1964), the brigade, in less than one month of fighting, had been reduced from 2,850 men to less than 1,200.

2. What general (and native son) was tasked with organizing this unit ? MG William F. "Baldy" Smith. Smith was born in St. Albans, Vermont.
 
The 1st Vermont Brigade. But it did include for a short time the 26th NJVI. The 1st New Jersey Brigade was also known by its namesake state. It included only regiments from NJ during the war and as far as I know was the only brigade to do so. It started with the 1st, 2nd, and 3rd NJVI and as time went on the 4th, 10th, 15th, 23rd, 40th NJVI and Hexamer's Battery A 1st NJ Artillery all served in the brigade. No other troops from another state, at any point during the war, were attached to the brigade. They were there at 1st Bull Run and marched at the Grand Review. At the time it was a source of pride for the state to maintain it's single state identity . We also had a 2nd NJ brigade that started with the 5th, 6th, 7th, and 8th NJVI but as time went on units from other states were added.

Edit - The Wikipedia article on the First Vermont Brigade refers to it variously as the "First Vermont Brigade," the "Old Brigade," and simply the "Vermont Brigade," so I am counting any of those as correct answers.

Obviously, there was no third part to the question, so no one is penalized for not giving a three-part answer.
