1. Richard Dale Owen (1810-1890).
2. 60th Indiana Volunteer Infantry.
3. He drank mislabeled embalming fluid.

3. What caused his death ? His death was caused by accidental poisoning, when a jug of embalming fluid was sent by mistake to Owens’s neighbor. The jug, labeled “medicated water,” was mistaken for mineral water, and Owen and his neighbor drank small quantities. Discovering their mistake, medical aid was sought; the neighbor survived, but Richard Owen did not. The incident occured March 24th, 1890.

Edit - As a couple of players pointed out, Owen served in two different regiments, the 15th Indiana and the 60th Indiana, during the Civil War. I am accepting either of those as a correct answer to the second part of the question.
