It was Jeb Stuart. I don't know where it was given to him.

Easy question as I am currently reading the 1930 biography Jeb Stuart by legendary author and U.S. Marine John W. Thomason. It was a carryover from his days at West Point and the Old Army.
 
JEB Stuart was given this nickname at West Point.
