Looks like Cozzens even got his name wrong in one article I found. Found this in the comments section of one of his articles.

Peter Cozzens

Bethesda Maryland June 9, 2014

I would like to thank Mr. McPhillips for pointing out my error. He is indeed correct - Howard Andrew Millet Henderson. I'm not sure why I had "Henry" on the brain when I wrote this piece; I knew better, but didn't catch my error.

In regard to the rank question, I have found most sources show him as Captain while discussing Cahaba, many other sources show an 1864 promotion to colonel.
In the same Cozzens piece, he goes on and writes:
Unfortunately for them, Colonel Henderson was no longer at Cahaba.

The rank question component leaves some wiggle room in interpretation. Because the question addresses the Cozzens article. I am changing my answer to colonel as that rank is found in the Cozzens piece found in the link below. I have found sources showing both captain and colonel ranks depending on what the time frame is. I always read up more on these trivia questions. Sometimes the more I read, the more confused I become.

ANSWER:
1. Rev. Henry / Howard A. M. Henderson, DD (15 August 1836 - 2 Jan 1912)
2. He was a former Confederate Colonel
3. During the war, Henderson served as Commandant of the Cahaba prisoner of war camp. During its 22 months of operation, more than 5000 Union soldiers were imprisoned there (June 1863 to April 1865.)

Edit - Various sources list Henderson's rank as captain, colonel, or lieutenant colonel. I accepted all three as correct answers to the question of what Henderson's rank was.

Stony subsequently submitted a source (below) in support of his contention that Henderson should be regarded as co-commandant, rather than commandant, of Cahaba Prison. While there is merit to this argument, I am accepting either commandant or co-commandant as a correct answer to the question of what position he held.
